1. What Bamboo Access is

Bamboo Access is a voluntary digital health support program offered by Bamboo Health as a participant in the Centers for Medicare & Medicaid Services (CMS) Innovation Center ACCESS model. It is an addition to the care you receive from your own providers. It is not insurance, not a Medicare Advantage plan, not a Medicare supplement, and not a substitute for medical care, diagnosis or treatment.

2. Who can participate

To participate you must:

  • Be enrolled in Original Medicare (fee-for-service) with Part B. People enrolled in a Medicare Advantage plan are not eligible.
  • Live in an area Bamboo Access serves.
  • Be able to take part in monthly outreach by phone, text message or secure message, on your own or with a caregiver's help.

A referral is not required. Most participants are referred by their care organization so that Bamboo Health can work alongside their primary care team, but you may enroll yourself.

3. Tracks, provider choice and the 90-day period

  • The program offers three tracks: Enhanced Cardiovascular-Kidney-Metabolic care, Cardiovascular-Kidney-Metabolic care, and behavioral health support. You may participate in more than one track at the same time.
  • You may work with more than one ACCESS provider, but only one provider per track. Two providers cannot serve you in the same track at the same time.
  • Your choice of provider for a given track remains in place for 90 days. After 90 days you may change providers for that track.
  • You may disenroll from Bamboo Access entirely at any time, for any reason, with no penalty and no effect on your Medicare coverage or benefits.

4. What we ask of you

Participation depends on engagement. We ask you to complete one check-in each month, respond to short questions between check-ins, and — in the Cardiovascular-Kidney-Metabolic tracks — take blood pressure and weight readings with the connected devices we provide and keep required lab work current with your provider. If we cannot reach you over an extended period, we may pause or end your participation and will notify you.

5. Connected devices

Any blood pressure monitor, scale or other device we provide is supplied at no cost for use during your participation, remains the property of Bamboo Health or its supplier, and should be returned or disposed of as we instruct when you leave the program. Devices transmit readings to your care team. They do not provide alarms, continuous monitoring or emergency response.

6. Cost

Participants have no financial responsibility for the Bamboo Access program: no copay, no coinsurance, no deductible and no charge for provided devices. Care you receive from your own providers is billed to Medicare as usual and normal cost sharing applies. Other organizations participating in the ACCESS model may bill the standard 20% Medicare coinsurance for their services; Bamboo Health does not.

7. Text messaging and calls

By texting a keyword to our short code or giving us your phone number, you agree to receive program messages and calls, which may be sent using automated technology. Consent is not a condition of receiving any healthcare service. Message frequency varies. Message and data rates may apply. Reply STOP to stop messages and HELP for help. Text messages are not a secure channel and must never be used to report an emergency.

8. Not for emergencies

Bamboo Access does not provide emergency or urgent care and does not monitor your information around the clock. If you are having a medical emergency, call 911. If you are in emotional distress or crisis, call or text 988 to reach the Suicide & Crisis Lifeline at any hour.

9. Your information

We use and share your health information as described in the Bamboo Access privacy notice, including sharing updates with your primary care provider and reporting to CMS as the ACCESS model requires. We do not sell your information.

10. Changes to these terms

We may update these terms as the program or the ACCESS model changes. We will post the updated version here with a new date and, where required, notify participants directly.
